Everything attached to your record at this clinic, filed in folders — lab results, ID, visit summaries, medications, immunizations, imaging, forms and what you signed. What you upload goes into the right folder and your care team is told.

Add to your record
Photos are fine: results from another doctor, a medication list, an immunization record, an imaging report. Pick what it is and it lands in the right folder; a person on the care team looks at each upload before it is filed. Your insurance card goes under Insurance.
Your whole record
You have the right to a copy of everything in your record. Request it here and it’s ready to download within 24 hours, usually much sooner, at no charge.
Sharing
To send files to another doctor, sign a release under My information › Sharing; it goes the same day. Family members you have given access see the folders you chose.
